All Leaders must fill out the Leadership Application before this form can be submitted. Please direct them to the link below so that you can complete this form.
All SFC Chapters will at some point be fundraising to support their mission in their own communities. Similarly, all SFC Chapters will need insurance for their events, small or large. This goes for all weekly events such as bible studies or outreaches as well as all larger, less often events such as rail jams or movie premieres.
Your chapter has the choice of doing this through SFC for a small percentage of funds raised or through another organization for a yearly fee.

To get your insurance set up please complete the questions below. For help with the participant/spectator numbers think of it like this: If you are holding a bible study and 10 people show up and they continue to show up for the entire season, then only put down 10 people. If you have a bible study and 10 people show up, and the next time 10 different people show up, and the same happens for the 20 weeks you're doing it then put down 200 people. Got it?
